Unfortunately Optima Family Health, Straight Medicaid, Southern Health CareNet, and VA Premier DO NOT cover Gardasil for women OVER the age of 18. Gardasil is available for purchase at $155.00 per injection**.

- Are 19 years old or older
- Do not have health insurance, or
- Cannot afford to pay for the vaccines;
You may be eligible for the program if all three of the following conditions apply:
- You reside in the United States and are age 19 or older*
- You have no health insurance coverage (Some examples of health insurance coverage include private insurance, HMOs, PPOs, college health plans, Medicaid, Veterans' assistance, or any other social service agency support.)
- You have an annual household income less than:
-- $20,800 for individuals
-- $28,000 for couples or
-- $42,400 for a family of four
However, Merck recognizes that sometimes exceptions need to be made based on a patient's individual circumstances. Individuals who do not meet these criteria may still qualify for the vaccine program if the patient has special circumstances of financial and medical hardship.***
